DESCRIPTION A 36-year-old woman presents with significant pain and swelling over the left axillary area, which she reports worsened over the last 3 years. The tenderness becomes more prominent with her menstrual cycle. The patient denies any other changes in her breast.

CASE PRESENTATION A skin nodule in the axillary skin on excision biopsy revealed invasive lobular carcinoma. CONCLUSIONS Carcinoma in accessory breast tissue is uncommon especially invasive lobular type. A high index of suspicion may avoid late diagnosis.
